2024 AREA SERVICES MEMBERSHIP
This subscription entitles the holder to unlimited Emergency Medical Service and additional ancillary services (when available) until March 31, 2025, subject to Terms and Conditions which are available upon request.

I understand that I am financially responsible for the services provided to me by AREA Services, referred to as "AREA", regardless to my insurance coverage. I request that payment of authorized Medicare or other insurance benefits be made on my behalf to AREA, or its billing agent for any services provided to me by AREA. I authorize and direct any holder of medical information or documentation about me to release to the Centers of Medicare and Medical Services and its carriers and agents, as well as to AREA and its billing agents, any information or documentation needed to determine these benefits or benefits payable for any services provided by AREA, both now and in the future. I also agree to immediate remit to AREA any payments that I receive directly from any source for the services provided to me.
